Health Plans for Chronic Disease
We provide Health Care Plans for Chronic Disease in a positive and caring environment. Our GP’s are fully equipped to plan and coordinate health care programs for patients suffering from chronic or terminal medical conditions and present patients with a structured approach to their care.



A ‘chronic medical condition’ is one that has been or is likely to be present for at least six months. Importantly, patients who are being managed by our GPs through this Health Care Plan service are also eligible for Medicare rebates for certain allied health services on referral from their GP.

Mental Health Care Plans
We are pleased to offer a Mental Health Care Plan service for patients seeking to become more involved in their health care.

A GP Mental Health Care Plan is a program made by our GP’s for treating mental health problems over time. Our practitioners will work with you to assess your mental health, work out what help you need, set goals and choose the treatment that is best for you.

Rebates are available through Medicare for mental health care services for eligible people.
